IMPAC

The Inner Melbourne Post Acute Care (IMPAC) program is a funded by the Department of Human Services with the aim of assisting people to recuperate from illness or injury immediately after discharge from a public acute hospital, rehabilitation or hospice facility. People discharged from private hospitals or publicly funded psychiatric beds are not eligible.

The program can also assist in a person's care, to divert them from acute public hospital admissions by brokering and coordinating the services required to support and maintain the person safely at home.

IMPAC purchases and coordinates a range of health and welfare services for eligible clients through our contracted providers. IMPAC does not duplicate existing programs (e.g. HACC or Community Health, or DVA) but augments these existing programs by "filling the gap" between discharge and the time when these programs can deliver services.

The IMPAC program covers the local government areas of Boroondara, Yarra, Melbourne and parts of Darebin (Fairfield, Alphington, Northcote and Thornbury).
